If you would like to schedule an appointment with Dr. Lin, please call our office at (408) 358-6580. After scheduling, please print New Patient Forms here, fill them out legibly, then either fax them to our office at (408) 358-6515 or bring them with you to your initial consultation, along with your insurance cards and a photo ID.
